This is a document drawn up between your manager, on behalf of SSG Support Services Group Ltd, and you, as an employee, to outline steps we can take to stop spread of Coronavirus (COVID-19). This information will be held confidentially and regularly reviewed, in partnership with you. You only need to tell us details about your Current health in relation Coronavirus (COVID-19). The document is not legally binding but will help us to agree, together, how to practically support you in your role and address any health needs related to Coronavirus (COVID-19). Where necessary, the company retains the right to request you to take preventative measures, such as undertaking a medical check-up and/or self-isolation for 14 days. These measures are in line with the government guidelines and will help us maintain a safe and healthy environment for you and your colleagues.
